NOTE: Registration is first-come first-serve and is complete only when both this form and payment are received. The registration fee is $30 per individual player AND includes a FREE DISC! It can be paid to your captain via cash or cheque (made to Fredericton Ultimate) on the first night of play. | ||||||||||
